Support Care

Because Activities of Daily Living can become a hurdle to living a happy and healthy life, our Support Care Services assist our patients, their families and or loved ones with Activities of Daily Living. These patients do not have a qualifying medical need for nursing services through Medicare, but can benefit from a needed support to assist them in their day to day activities. We provide support care under a state and federally funded program, through commercial insurance or private pay. We can provide any or all of the following services depending on the patient’s needs.

Our services are structured on the Medicare Model, our field staff is supervised by trained nurses of Legacy Home Health Care which is a Medicare Home Health and Hospice certified agency, if any concern arises, our care givers report to a clinician trained to provide care in the home… In other words, your loved ones are in good hands.

Our Support Care Services’ mission is to provide the premium support the members of our community who need to remain in their home in a safe and independent manner.
