Hospice Care

When you or your loved one is facing the final stages of life, our caring hospice professionals provide the emotional, spiritual and medical support you need. The goal of our program is to help patients live comfortably, with dignity, when a cure is no longer possible. The earlier a patient is referred to hospice, the better. It is much more comfortable for a patient to have a team in place and to manage symptoms and coordinate care. Your physician and the hospice physician will help determine when a referral is needed.
Anyone can make a referral to hospice at any time.

Your care team consists of you and your family, your doctor, the hospice physician, your nurse, a hospice aide, social worker, volunteers, spiritual support and bereavement support. Let us introduce you to your team, and care for you, our patient, your family member, your loved one. We are contracted with a durable medical equipment company to provide you with all you may need, such as a hospital bed, as well as your medications that are for your hospice diagnosis. Home hospice services are on call 24 hours a day seven days a week.
